Forms
The following forms have been developed to assist faculty with Risk Management in service-learning courses. Faculty teaching internship courses may not find that these forms apply to their courses; additional resources for internships will be available soon. Faculty instructing courses with student teaching or clinical experiences should consult with the appropriate offices in the School of Education and School of Nursing regarding procedures for Risk Management.

Forms for Faculty:
Community-Based Learning Agreement (DOCX): REQUIRED FOR ALL CBL COURSES. Outlines the nature and scope of the agreement between the University and Community Organization. This document must be signed by the Community Organization and a University Dean. The signed form should be submitted to the CBL Coordinator one month before the first day of class. The Project Agreements executed with Community Organizations will be in effect for the duration of each Project, up to one year.
Site Visit Form (DOCX): A suggested list of items to discuss with the community partner organization to outline the details of the project and ensure a smooth process and a safe experience for students.
Additional Resources For Faculty:
Orientation to Community-Based Learning: A 30-45 minute orientation for your students covering: an introduction to Community-Based Learning, appropriate behavior for working with community partner organizations, safety and emergency procedures, & signing of liability waivers. SXU staff will conduct the orientation during one of your class sessions. Timeline: Register for an orientation for your class by contacting the CBL Coordinator at least 2 weeks prior to the first day of class.
Risk Management Principles for Faculty (DOCX): An overview of important risk management guidelines for CBL courses, and a corresponding list of resources to aid you in implementation.
Suggested Timeline for Service-Learning Courses (DOCX): this suggested timeline helps to provide faculty with a timeline of when different aspects of the Risk Management process should be handled.
Suggested topics for Orientations from Faculty and Community Organizations (DOCX): In addition to the Orientation to Community-Based Learning, it is strongly recommended that Faculty members orient students about topics specific to their course(s) and that students receive an orientation (either on-campus or on-site) to the Community Organization to which they will be assigned. A list of recommended topics to cover in these orientations by both faculty members and Community Organizations is attached.Resources for Background Checks, VIRTUS Training, TB tests, etc. (DOCX): Some community partner organizations will require that students to complete certain screening before volunteering with their organization, such as a criminal background check, VIRTUS training, a TB test, etc. Click here to see information on resources to help students complete any requirements they may have.
Forms for Students:
Liability Waiver (DOC): This form must be signed and submitted during the Orientation to Community-Based Learning by the end of the third week of classes.
Hours Log (DOCX): If your instructor requires you to document your hours on site, you may use this form to do so.
CBL Learning Agreement (DOCX): If your instructor requires you to submit a CBL Learning Agreement, you may use this form to do so.
