Verification Forms

If your Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification, Saint Xavier University must gather additional information about you and your parents (or spouse), as required by the U.S. Department of Education (34 CFR, Part 668), in order to process your financial aid.

2013-2014 Verification Forms

Please note: If selected for 2013-2014 verification, you do NOT need to complete all of the forms below. The necessary documentation varies by student and is detailed in the email (sent to your SXU email account) by the Office of Financial Aid. Please refer to your email, or contact the Office of Financial Aid, to eliminate the possibility of completing more documentation than required.

2012-2013 Verification Forms

If the Office of Financial Aid is requesting Income Verification for you or your parents (or spouse), you can satisfy this requirement by completing the Link to the IRS within the online FAFSA application or by ordering your Tax Transcript from the IRS. To make the process easier, you can view step-by-step instructions (PDF) on how to complete the IRS link or obtain your tax transcript.

Appeal Forms

Appeal forms are for any student who has a special circumstance or a dependency status issue. Use the following links to complete the appeal process. Return the documents to the Office of Financial Aid.
