Financial Aid Forms

Verification Forms

If your Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification, Saint Xavier University must gather additional information about you and your parents (or spouse), as required by the U.S. Department of Education (34 CFR, Part 668), in order to process your financial aid.

2014-2015 Verification Forms

Please note: If selected for 2014-2015 verification, you do NOT need to complete all of the forms below. The necessary documentation varies by student and is detailed in the email (sent to your SXU email account) by the Office of Financial Aid. Please refer to your email, or contact the Office of Financial Aid, to eliminate the possibility of completing more documentation than required.

Appeal Forms

Special Circumstance Appeal Forms can be used by students to report unusual circumstances that may impact their ability to pay for their education at Saint Xavier University. These circumstances can be conditions that have reduced the family's income for the current year (e.g. loss of job; reduction in wages; loss of one-time income) or extraordinary expenses that provide the family with less disposable income (e.g. significant medical expenses; elementary/secondary private school tuition for siblings).

Dependency Override Forms are available to students who are determined, by the Free Application for Federal Student Aid (FAFSA), to be classified at dependent students, but meet limited conditions to be considered an independent student.

To obtain Special Circumstance Appeal or Dependency Override Forms, please contact your Financial Aid Counselor, who will assist you in evaluating your eligibility for consideration.
