New Student Immunization Records
All incoming undergraduate students are required to submit a compliant Certificate of Immunity by Friday, February 17, 2012. For a complete description of what is needed, download the New Student Immunization form (PDF).
Student Health Insurance
The University requires that all full- and part-time undergraduate students enrolled in any University program be covered under a health insurance plan.

- Chronic illness management (Nebulizer available for asthmatic patients)

Children Lead Screening Program
The Chicago Department of Public Health offers lead screening at a variety of sites around the Chicago area. However, none are scheduled at the Health Center at this time.
