The form is provided in a Word document. You may either print out a blank form and hand-write in it, or simply type into the sections on the form provided here. If you choose to type directly on the form, it will act like any Word document. Place the cursor wherever you would like to type information. The boxes on the form are designed to provide sufficient space for most needs. However, if you need more space than the form allows, please continue your comments on a separate sheet and attach them to the final printed form. You may save the form at any time. You may save multiple versions by assigning each version a different document name.
Each employee should fill out a self-evaluation. The employee may either type into the form provided, if he or she has access to a computer, or hand write on a pre-printed blank copy.
If you are a supervisor with multiple employees with similar jobs, you may want to fill in the common sections and save one copy for each employee (name them something like Appraisal 05 Jane Doe.doc, Appraisal 05 John Smith.doc, etc.). You can then go back into each employee’s individual form to complete the parts that are particular to that employee without having to retype the common components.
The final appraisal form for each employee must be printed so that hand-written signatures can be applied. The completed, printed, signed, form should be sent to Employee Services for inclusion in the employee’s official file.
Remember that the performance appraisal is a confidential document, and care should be taken to ensure the privacy of the document in both printed and electronic forms.
