Do I need to make an appointment?
We can only guarantee an efficient visit with one of our providers if you have scheduled an appointment in advance. Walk-in appointments of SXU students are accepted; however, there might be a slight wait before seeing a provider. Please note that SXU students are our first priority. Can only SXU personnel become patients at the Health Center?
No. The Center extends its services to include the residents of the surrounding Chicago community.
Which insurances do you accept?
The only insurance we accept is Associated Insurance Plans, the student plan offered through SXU. Some laboratory tests sent to Quest Diagnostics may also be billed to insurance companies directly from Quest. Please ask a nurse for further clarification.
What are the payment options of a Health Center visit?
Payments of cash, personal check or credit cards are accepted from community members and SXU students, staff and faculty.
How do I request my records?
Patient records can be picked up by the patient or another designated person. Records can also be mailed or faxed upon the patient's request. A Patient Record Release form (PDF) needs to be completed and submitted to the Health Center at least 24 hours before records are ready to be picked up or sent out. The Patient Record Release can be downloaded from this Web page or picked up in person. There is no charge for this service.
Are there doctors on staff at the Health Center?
The Health Center has Family Nurse Practitioners and a Registered Nurse. The Center has a collaborative relationship with a local Family Practice Physician.
Where is the Health Center located?
The Health Center is located in the Northwest corner of Parking Lot 4, behind Regina Hall. There is a large radio tower directly behind us. We are in a beige modular unit. Campus map (PDF).
