Our staff supports the academic mission of student learning by assisting you in leading a healthy life. In order for us to best serve you, it is necessary for us to receive proof of immunizations and the student's medical history. Please know that all SXU students are required by Illinois Law to have a completed health form (including an immunization history) on file with the Health Center.
The State of Illinois Law requires that all students born after January 1, 1957, show proof of two measles (rubeola) vaccinations given after the student’s first birthday, one mumps and one rubella vaccinations. The State of Illinois law also requires a current (within the last 10 years) Tetanus-Diphtheria immunization.
Due to the recent mumps outbreak in the Midwest, the American College Health Association (ACHA) changed the recommendations for the mumps vaccine. Based on the new recommendation, SXU is requiring two MMR (measles, mumps, rubella) vaccines.
The Certificate of Immunity and Medical History forms will be mailed to students' homes during the summer before beginning classes. Forms will also be available to download from this Web page.
