Student Organization Forms

Please note that forms must be submitted in a timely manner (see each form for specifics).

Student Organization Funding Request (DOC)

Initial funding requests are due Tuesday, November 20 by 4:30 p.m.  Organizations that fail to participate in the student organization funding request process will not be funded for the spring 2008 semester.

Event Permit/Speaker Clearance Proposal (PDF)

All student organizations must turn in a completed form signed by their advisor for all events including table reservations in the SXU Diner, fundraisers, outdoor events, regular meetings, etc. This form must be submitted at least three weeks prior to the planned event.

Dining Service Requisition (PDF)

All catering on campus must utilize Chartwell’s Dining Services using this form.

Check Request (DOC)

Requests for checks must be submitted at least 2 weeks before needed. Checks will be cut only for expenditures related to approved events when funds are available in the organization’s account and all supporting documentation is attached.

Purchase Order Requisition (XLS)

To be used for bus reservations in addition to T-shirt, poster, and novelty orders, depending on the vendor.

Performance Agreement (PDF)

To be filled out by anyone contracted by your organization to provide a service that does not have an external contract (whether or not they are being paid).

Re-registration Form (PDF)

All registered student organizations must submit this completed form to the Department of Campus Life (L-103) at the start of each Fall semester.
