Call for an Appointment
- Please call 773-298-3712 during our hours of operation (see below) to schedule an appointment.
- We often have same-day appointments available, but we ask that you call ahead so that you do not have to wait unnecessarily. Walk-in appointments will be seen at the next available appointment, which may be the same day.
- We ask that anyone with COVID-19 symptoms/positive COVID-19 test should NOT walk in for an appointment. Please call ahead. Depending on the symptoms you are having we may test you before you come in to the Health Center and/or we may escort you back to an exam room so that you are not close to other people in the waiting room.

Insurance
- Insurance is accepted at the SXU Health Center. Please see our list of in-network insurances.
- We recommend that you contact your insurance carrier prior to your visit to the Health Center to verify coverage of the specific service you are seeking.
- To verify coverage, call the member service phone number on the back of your insurance card in order to confirm your benefits, coverage, copay and deductible for the specific service.
- If you do not have coverage for the service you are seeking, please call the SXU Health Center to inquire about our self-pay rates.
Self-Pay
- We offer reasonable self-pay rates for out-of-network and uninsured adults and children.
