Welcome to The SXU health center's Patient Portal

The SXU Health Center's Patient Portal allows our patients to communicate securely with providers and office staff. Our Patient Portal is a great option for ALL of our patients, including SXU students, SXU staff/faculty, and community members. Our portal is completely HIPAA compliant.

When you have an account you have access to numerous features, including:

Creating an Account

If you had lab tests completed at the SXU Health Center a portal account may have already been set up for you at your appointment. However, if you do not have an account please follow the instructions below:
SXU Students and Staff Community Members
  • Create your own username
  • Enter your SXU email address
  • Enter your 7 digit SXU ID number (this includes a leading '0')
  • Enter your name exactly as it is on University records (e.g. Kathleen Smith, not Kathie Smith)
  • Enter your birth date
  • After you submit your request, you will receive an email with a link you need to follow to create your password and access your account
  • Create your own username
  • Enter your email (this should match the email address that you put on your registration form during your visit)
  • Enter the 7 digit ID number that was given to you at your appointment (if you are unsure, please call the Health Center for assistance at 773-298-3712)
  • Enter your name exactly as it was entered into our system when you made your appointment (e.g. Kathleen Smith, not Kathie Smith)
  • After you submit your request, you will receive an email with a link you need to follow to create your password and access your account

Instructions for Immunization Upload

All SXU students taking more than 6 credits per semester (at any point in their time at SXU) must comply with the state immunization requirements or they will have their registration blocked for subsequent semesters.
