Verification Forms

If your Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification, Saint Xavier University must gather additional information about you and your parents (or spouse), as required by the U.S. Department of Education (34 CFR, Part 668), in order to process your financial aid. Verification documents must be received and processed by SXU, no later than 120 days after the student's last date of enrollment for the award year. Failure to meet this deadline will result in the student's inability to participate in Federal Work Study employment or cessation of employment, and cancellation of all need-based Federal and State awards, including Pell Grant, SEOG, Perkins Loan, Subsidized Stafford loan, and MAP grant. Please submit all required documents as soon as possible to prevent delays.

Please note: If selected for 2024-2025 verification, you do NOT need to complete all of the forms below. The necessary documentation varies by student and is detailed in the email (sent to your SXU email account) by the Office of Financial Aid and listed in your Required Documents on mySXU Self-Service. Please refer to Self-Service or your email, or contact the Office of Financial Aid, to eliminate the possibility of completing more documentation than required.

Income Verification:

If the Office of Financial Aid is requesting Income Verification for you (or your parents or spouse), you must provide 2022 tax data directly from the IRS. There are several methods for doing so; please complete one of these options:

  • Link your (or your parents' or spouse's) taxes directly from the IRS into your FAFSA application by logging back into your FAFSA, selecting the option to Make Corrections, and utilizing the IRS Data Retrieval Tool. This is the most efficient method of meeting the Income Verification requirement.
  • Obtain a 2022 IRS Tax Return Transcript by going to the IRS website and selecting the option to Get My Tax Record. Please note: only Tax Return Transcripts will be accepted (not Tax Account Transcripts). Download or request your transcript by mail, then provide a copy of this transcript to the Office of Financial Aid to meet the Income Verification requirement.
  • Provide a signed photocopy of your (or your parents' or spouse's) 2022 Federal Tax Return and applicable schedules. If your parents (or spouse) filed separate tax returns, you must provide tax data for each person.

Verification of Non-Filing:

If the Office of Financial Aid is requesting Verification of Non-Filing for you, your parents or your spouse, you must provide confirmation of non-filing from the IRS to demonstrate that 2022 Federal Income Tax Returns were not filed. A Verification of Non-filing Letter can be obtained from the IRS by going to the IRS website and selecting the option to Get My Tax Record. Provide a copy of the Verification of Non-filing Letter, for tax year 2022, to the Office of Financial Aid to meet this verification requirement.

Appeal Forms

Special Circumstance Appeal Forms can be used by students to report unusual circumstances that may impact their ability to pay for their education at Saint Xavier University. These circumstances can be conditions that have reduced the family's income for the current year (e.g. loss of job; reduction in wages; loss of one-time income) or extraordinary expenses that provide the family with less disposable income (e.g. significant medical expenses; elementary/secondary private school tuition for siblings).

Unusual Circumstance Forms are available to students who are determined, by the Free Application for Federal Student Aid (FAFSA, to be classified a dependent student, but meet limited conditions to be considered an independent student based on a unique situation.

Budget Appeal Forms are available to students whose Cost of Attendance budget is not sufficient to meet their education-related costs and additional borrowing capacity is desired.

To obtain Appeal or Override forms, please contact the Office of Financial Aid. A Financial Aid Counselor will assist you in evaluating your eligibility for consideration.
